About The Position

This role involves supporting and implementing the Mission, Vision, and Core Values of Beacon Behavioral Hospital, and upholding its 5 Commandments for Exceptional Customer Service. The LPN will participate in the planning, coordinating, and documenting of total patient care in conjunction with a multidisciplinary treatment team. This includes accurately reporting and recording patient conditions, preparing daily clinical/progress notes, and documenting monthly recertifications/Master Treatment Plan Updates. The LPN will also assist patients with daily living activities, teach self-care techniques, and educate/supervise other healthcare personnel. Responsibilities extend to completing clinical audits, implementing physician orders, maintaining awareness of Medicare reimbursement criteria, and ensuring timely and accurate documentation. The role also involves participating in case conferences, facilitating nursing psych-education groups, completing nursing health screens for new admissions, communicating issues to physicians, and contributing to nursing Performance Improvement initiatives. The LPN will also participate in the development and implementation of the Plan of Care.

Requirements

  • Graduate of an accredited school of nursing.
  • Current Practical Nurse licensure in the state of operation.
  • Minimum of 1 year experience as a Licensed Practical Nurse.
  • Certification in Basic Life Support required.
  • Certification in non-violent crisis intervention.

Responsibilities

  • Participates in the planning, coordinating and documenting total patient care in conjunction with the multidisciplinary treatment team.
  • Accurately reports and records the patient’s condition and care, including signs and symptoms which may be indicative of change.
  • Prepares clinical/progress notes daily and incorporates into the clinical record weekly, and documents monthly recertifications/Master Treatment Plan Updates in coordination with the multidisciplinary treatment team.
  • Assists the patient with activities of daily living while teaching appropriate self-care techniques.
  • Educates and supervises other healthcare personnel in providing continuity of care.
  • Completes all clinical audits as delegated by Program Director. Assures all deficiencies are corrected and compliant with standards within a timely manner.
  • Initiates and sustains the implementation of all physician orders.
  • Remains cognizant of Medicare reimbursement criteria, current charting needs and dates of recertification of all patients under his/her care and supervision.
  • Completes, maintains and submits accurate and relevant clinical notes.
  • Participates in case conferences, in-services, and other programs and meetings as needed.
  • Facilitates nursing psych-education groups as needed.
  • Assures complete and timely documentation of each group facilitated.
  • Complete Nursing Health Screen for every new admission.
  • Communicate any issues/concerns with physician.
  • Assist with nursing Performance Improvement contributions to problem identification, investigation, actions and monitoring of resolutions in effecting patient care and customer satisfaction systems.
  • Participates in the development and implementation of the Plan of Care in coordination with other members of the multidisciplinary treatment team.
