About The Position

This role involves providing direct patient care in an outpatient setting, focusing on medication management, patient education, and collaboration with a multidisciplinary treatment team. The LPN will be responsible for clinical duties, record keeping, and ensuring patient comfort and safety.

  • Current LPN in Alabama.

Responsibilities

  • Respond to consumers’ requests for medication refills and review charts for treatment compliance.
  • Administer medication observing the seven rights of medication administration and ordering medications to the pharmacy.
  • Perform venipuncture for laboratory requests, drug screens, TB skin tests, etc., monitor lab results, and report all abnormal findings to the physician.
  • Provide individual, family, and group education regarding the consumer’s psychiatric illness and medication.
  • Monitor consumers for potential side effects of medications and report findings to the physician/CRNP/RN.
  • Communicate effectively with the physician regarding the consumer’s physical and mental condition.
  • Follow up on missed injections and document appropriately in the consumer’s record.
  • Complete admission procedures for consumers and update accordingly, including medical checklist, AIMS, vital signs, Medication List, and MARS if applicable.
  • Document on, update, and reconcile MARS if applicable.
  • Interact with consumers and evaluate needs to ensure proper information is reported during staffing with the treatment team.
  • Conduct infection control training for staff and consumers when indicated.
  • Provide HIV education when necessary.
  • Provide nursing coverage to other Adult Outpatient programs within AltaPointe when necessary.
  • Interface with the multi-disciplinary team and actively and cooperatively participate in the development of the interdisciplinary treatment plan.
  • Provide emergency care as needed (CPR, PMAB, Emergency Medications), according to AltaPointe Policy & Procedures.
  • Demonstrate the ability to recognize the elements of a crisis state and know how to deescalate or resolve the situation.
  • Adhere to professional code of ethics.
  • Effectively demonstrate the application of adult specific competencies.
  • Seek clinical supervision and consultation needs.
  • Accept and employ suggestions for improvement.
  • Actively work to enhance clinical skills.
  • Document in a timely fashion per AltaPointe policy.
  • Document in a clear, concise manner, types of consumer problems, treatment goals, and services provided.
  • Ensure documentation shows that forms, notes, logs, and other applicable written information are completed promptly, accurately, and updated when appropriate.
  • Document legibly.
  • Meet AltaPointe productivity standards.
  • Treat consumers with care, dignity, and compassion.
  • Respect consumer’s privacy and confidentiality.
  • Be pleasant and cooperative with others.
  • Assist consumers and visitors as needed.
  • Ensure personal values don’t inhibit ability to relate and care for others.

  • Be gentle and calm to consumers and families.

  • Actively participate in AltaPointe committees as required.
  • Complete assigned tasks in a timely manner.
  • Access appropriate community resources according to consumer needs.
  • Comply with AltaPointe policies & procedures.
  • Maintain current State license and requirements for renewals (i.e. CEU’s), and attend required in-services and workshops.
