About The Position

Sanford Health is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) or Med Aide to join their cottage hospice team. This role involves providing comfort-focused, patient-centered care to individuals and families during a significant life stage. The position requires close collaboration with an interdisciplinary team to support symptom management, emotional comfort, and patient dignity. The LPN will participate in the implementation and evaluation of patient/resident care for stable and predictable patients/residents of all ages in the home care setting. This includes demonstrating knowledge of nursing principles, assessing patient status, interpreting information for age-specific needs, and providing appropriate care. The role also involves ensuring patient/resident health, comfort, and safety, understanding medication effects, patient education principles, and common safety hazards. Skills in administering treatments and medications are essential.

Requirements

  • Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
  • Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
  • Obtains and subsequently maintains required department specific competencies and certifications.
  • Basic Life Support (BLS) for health care providers required.

Nice To Haves

  • One year of professional nursing experience preferred.
  • If position requires IV therapy/management skills, the following is required in North Dakota: graduation from a Board approved AASPN program where IV therapy is part of the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing.
  • Depending on location, a valid drivers license may be required.

Responsibilities

  • Participates in the implementation and evaluation of patient/resident care for stable and predictable patients/residents of all ages in the home care setting.
  • Demonstrates knowledge of nursing principles for the patient/resident in the relevant setting.
  • Performs testing to assess data reflective of the patient's/resident's status and interprets appropriately the information needed to identify each patient's/resident's requirements relative to the age-specific needs, and to provide the care needed.
  • Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in the home care setting.
  • Ensures the health, comfort and safety of patients/residents.
  • Applies knowledge of the principles and skills needed for nursing to provide patient/resident care and treatment.
  • Applies knowledge of medications and their effects on patients/residents.
  • Applies knowledge of patient/residents education principles to develop a self-care program.
  • Applies knowledge of common safety hazards and precautions to establish a safe environment, specific to the home setting.
  • Administers a variety of treatments and medications as directed.
