LPN Med Care Manager
About The Position
As an LPN Medication Nurse at Sunrise Senior Living, you provide licensed nursing support through safe medication administration, health monitoring, and accurate clinical documentation. You help protect residents by preventing medication errors, identifying health changes early, and following clinical and regulatory standards. The role includes administering medications and treatments, documenting care, monitoring residents, reporting concerns, conducting Service & Health Updates, transcribing physician orders, and maintaining medication carts and narcotic counts. Requirements
- Active LPN license in good standing.
- CPR and First Aid certification.
- Strong assessment and documentation skills.
Nice To Haves
- Experience in senior care or medication administration.
Responsibilities
- Administer medications and treatments according to physician orders.
- Follow the Six Rights of Medication Administration.
- Accurately document in the MAR/TAR in real time.
- Monitor symptoms and administer ordered medications for symptom relief.
- Promptly report medication refusals or adverse reactions.
- Conduct Service & Health Updates (SHUs).
- Transcribe physician orders and support care plan updates.
- Observe and report physical, emotional, or cognitive changes.
- Maintain medication carts, complete narcotic counts, and uphold compliance standards.
