LPN Med Care Manager
About The Position
The LPN Medication Nurse provides licensed nursing support to senior living residents through safe medication administration, health monitoring, and accurate clinical documentation. The role is important in preventing medication errors, identifying health changes early, and protecting resident safety through clinical precision and regulatory adherence. The nurse administers medications and treatments according to physician orders, monitors symptoms, documents care, reports refusals and adverse reactions, conducts Service & Health Updates, transcribes physician orders, and supports care plan updates. The nurse also maintains medication carts, completes narcotic counts, and upholds compliance standards.
Requirements
- Active LPN license in good standing.
- CPR and First Aid certification.
- Strong assessment and documentation skills.

- Experience in senior care or medication administration.
Responsibilities
- Administer medications and treatments according to physician orders.
- Follow the Six Rights of Medication Administration.
- Document accurately and in real time in the MAR/TAR.
- Monitor symptoms and administer medications as ordered for symptom relief.
- Promptly report medication refusals or adverse reactions.
- Conduct Service & Health Updates (SHUs).
- Transcribe physician orders and support care plan updates.
- Observe and report physical, emotional, or cognitive changes.
- Maintain medication carts and complete narcotic counts.
- Uphold compliance standards and help protect resident safety.
