About The Position

Sage Group HealthCare Staffing is seeking Travel LPN LTC for a 13-week travel/contract position in Traverse City, MI. This role is open to both local and traveler candidates. The LPN will prescribe and adhere to facility protocols, demonstrating exceptional care, passionate service, and unexpected luxury. Under the supervision of an RN, the LPN assumes responsibility and accountability for a group of residents/patients for a shift of duty, providing nursing care through assessment, implementation, and evaluation of the plan of care. The LPN must adhere to standards of care, manage the environment for safety, and supervise nursing assistants and helpers, following all facility policies, procedures, and the State Nurse Practice Act, including JCAHO standards where applicable.

Requirements

At least two years of recent/current SNF/LTC or Med Surg experience
At least 1 travel contract completed
Covid Vaccination Proof or exemptions
LPN License for State of Michigan
BLS Certification from AHA

Responsibilities

Prescribes and adheres to facilities protocols and kindness in all personal and professional interactions while on duty in and around the facility.
Demonstrates great customer service by embodying facilities three core principles of Exceptional Care, Passionate Services and Unexpected Luxury in all of their job duties and responsibilities.
Under the supervision of RN, assumes responsibility and accountability for a group of residents/patients for a shift of duty.
Provides nursing care through assessment, implementations, and evaluation of the plan of care.
Adheres to the standards of care for the area, manages the environment to maintain resident/patient safety, and supervises the resident/patient care activity performance by nursing assistants and helpers.
Follows all facilities policies and procedures.
Performs duties as defined by the State Nurse Practice Act.
Follows where applicable JCAHO standards.
