About The Position

Signify Health, part of CVS Health®, is seeking a Part-time Physician to provide In-Home Health Evaluations. This role involves engaging with patients in their homes to bridge gaps in care and build trusted relationships. The goal is to provide flexible, comprehensive, and personalized health evaluations directly to patients. The visits, lasting approximately 45 minutes, include a medication and medical history review, a physical evaluation, and potentially diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment plans. The primary workplace is the patient's private residence, which can vary in conditions. Candidates must be comfortable working in diverse home environments, which may include pets, secondhand smoke, and other airborne allergens.

Requirements

Active, unrestricted license(s) in coverage area(s).
Able to cover the primary locations: Traverse City (Counties: Antrim, Benzie, Grand Traverse, Kalkaska, Lake, Leelanau, Manistee, Mason, Missaukee, Osceola, Wexford).
Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants.
