About The Position

Signify Health, part of CVS Health®, is seeking a Full-time Physician to provide In-Home Health Evaluations. This role involves engaging with people in their homes to help bridge gaps in care and build trusted relationships to improve health outcomes. The physician will conduct comprehensive health evaluations, including medication and medical history reviews, physical evaluations, and potentially diagnostic tests. The role utilizes an iPad with an intuitive clinical workflow and does not require prescribing medication, ordering lab tests, or altering current treatment regimens. The primary workplace is the private residence of health plan members, which can vary in conditions and may include household pets, secondhand smoke, and other airborne allergens. Requirements

Active, unrestricted license(s) in coverage area(s)
Able to cover the following primary locations: Traverse City (Counties: Antrim, Benzie, Grand Traverse, Kalkaska, Lake, Leelanau, Manistee, Mason, Missaukee, Osceola, Wexford)
Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants.

Responsibilities

Provide In-Home Health Evaluations.
Engage with people in the comfort of their own homes.
Bridge gaps in care.
Build trusted relationships to make people healthier.
Bring flexible, comprehensive and personalized health evaluations right to people’s front doors.
Conduct visits that last about 45 minutes, allowing time to connect one-on-one and answer health questions.
Perform medication and medical history reviews.
Conduct physical evaluations.
Perform additional diagnostic tests if ordered by the person’s health plan (i.e., diabetic eye exams, spirometry, etc.).
Use an iPad and an intuitive clinical workflow for each evaluation.

Be part of efforts to visit millions of people nationwide with the goal of providing connections to the right care for people’s unique needs.
Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
Be flexible to travel locally and within licensed states, as needed.
Conduct virtual visits as needed.
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.).
