About The Position

Sage Group HealthCare Staffing is seeking LTC LPN/LVN for a travel/contract position in Claremont, NH. This is a full-time, 13-week contract position for a Long-Term Care unit. The role involves observing residents, administering medications and treatments, documenting care, and supervising LNAs. The position requires a willingness to float and has a schedule of 48 hours per week, typically with three 16-hour shifts. Holiday coverage may be required. Travelers must reside beyond a 50-mile radius of the facility and have their own reliable transportation, as ride-sharing or public transport is not available at this location. Covid vaccination is required, and declinations/exemptions are not accepted.

Requirements

Reside beyond a 50-mile radius of the facility location.
Recent/Current LVN/LPN experience (recent within 6 months): 3 years
LTC experience
Must be willing to float
Covid Vaccination Required
Traveler must have their own reliable transportation
LPN/LVN License for State of NH or compact
BLS Certifications from AHA

IV Certification
Rural/small facility experience
Prior travel or contract assignment experience

Responsibilities

Observe each resident on a continuing basis alert to changes in mental and physical well-being; notifies the Supervisor of pertinent observations and discusses the need to notify physician.
Receive Resident Status Report from off-going shift and give on-coming shift a detailed report of each resident’s status and events of the shift.
Accurately receive, transcribe, and carry out physician’s orders.
Administer medications and treatments as ordered by the physician and make proper documentation via a computerized system of MARs/TARs and electronic documentation.
Order and maintain all pharmaceuticals for the units.
Supervise LNAs in accordance with licensing requirements under the New Hampshire Board of Nursing.
Assign and measure nursing staff resident care consistent with abilities, job description and scope of practice and supervise the care given.
Assist direct-care staff with nursing care as necessary and as time allows.
