About The Position

Tryfacta is seeking an LPN – Long Term Care (LTC) for our client in Vestal, NY 13850. This is a 16-week contract assignment. The role involves administering medications and treatments, performing medication passes, monitoring resident conditions, completing documentation, providing nursing care, supervising CNAs, and supporting residents with various care needs including skilled nursing, rehabilitation, memory care, and wound care.

Requirements

• Active New York State LPN license required at the time of submission.
• No license hits allowed.
• Minimum of 1 year of Long-Term Care (LTC) / Skilled Nursing experience required.
• BLS certification required.
• Must be able to pass a medication administration (med pass) test and clinical observation.
• Strong attendance and punctuality required.
• Must be dependable and able to work in a fast-paced environment.
• Must be flexible with scheduling needs.

Responsibilities

• Administer medications and treatments as prescribed.
• Perform medication passes accurately and safely.
• Monitor and report changes in resident condition.
• Complete timely and accurate nursing documentation.
• Provide nursing care in accordance with facility policies.
• Supervise CNAs on assigned units.
• Provide care to residents with Alzheimer's disease and dementia.
• Support residents receiving skilled nursing, rehabilitation, memory care, and wound care services.
