About The Position

This position is for a Licensed Practical Nurse (LPN) on the night shift in an Assisted Living and Memory Care Unit. The LPN will provide nursing care for residents, offering supportive and restorative care, and collaborating on resident service plans. This role involves direct resident care, documentation, and medication management under supervision.

Requirements

• Graduate of accredited School of Nursing.
• Current LPN license
• CPR certification

Nice To Haves

• 1-2 years clinical experience in a hospital or similar health care setting preferred.

Responsibilities

• Contributes to resident care and service plans by collecting data based on age, needs, and abilities, and participating in care planning meetings.
• Communicates and collaborates effectively with residents, families, colleagues, and other members of the health care team to meet resident needs.
• Provides education, supervision, and counseling to residents and families regarding Assisted Living procedures and other care needs.
• Documents accurate, pertinent, and comprehensive information in a timely and concise manner in the client service record.
• Conducts supervisory visits of resident care assistants, companions, and homemakers as required.
• Participates in educational programs, inservices, meetings, and workshops, and utilizes current literature to enhance professional skills and knowledge.
• Under supervision of SALSA/RN DESIGNEE and in collaboration with physicians, pharmacists, and outside agencies, will be responsible for oversight of medication management including but not limited to ordering, storing, reconciling, and preparing medications for administration and/or CNA prompt.
• Performs other duties as assigned.
