About The Position

This position is for an LPN II at Centra Specialty, a 26-bed Long Term Care Hospital specializing in caring for medically complex patients requiring extended hospitalization and advanced clinical care. Typical cases include individuals recovering from critical illness who need ongoing respiratory support, complex wound care, intravenous therapies, cardiac monitoring, dialysis, and comprehensive rehabilitation services. The unit utilizes the LEAF system for patient monitoring and provides bedside dialysis treatments using TABLO dialysis machines. The LPN coordinates and manages care for assigned patients of all ages, collaborates with the interdisciplinary team, adheres to Centra's best practices, creates a safe patient environment, and coordinates care processes to achieve desired outcomes. Requirements

  • Licensed as an LPN in the Commonwealth of Virginia.
  • Current BLS certification.

Responsibilities

  • Provide patient care under the direction or supervision of a licensed medical practitioner, a professional registered nurse, or other licensed health professional, as authorized by the Health Regulatory Boards.
  • Coordinate and manage the care provided to assigned patients of all ages based on the unit care delivery model, requiring professional knowledge and consideration of specific patient conditions and treatment.
  • Collaborate with the interdisciplinary team to deliver high-quality patient care.
  • Adhere to a culture reflective of Centra’s best practices for quality and service.
  • Create and maintain a safe patient environment.
  • Coordinate various care processes that are necessary to achieve desired outcomes.
