About The Position
Empath Home Health, a member of Empath Health, is seeking a caring PRN Home Health LPN to provide individualized care in the Ft. Myers area of Lee County. The role delivers home health nursing services in accordance with agency policies and procedures, state and federal regulations, and each patient’s plan of care, under the direction and supervision of a Registered Nurse. Empath Home Health provides skilled nursing and therapy services that help patients recover and maintain independence at home, along with personal care support as part of a connected continuum of care. The schedule is PRN, with 5–15 or more home health visits per week. Weekend and holiday rotations are required. Requirements
- Active Florida LPN license.
- Graduate of an approved nursing program.
- 1–2 years of LPN experience.
Nice To Haves
- Home health care experience preferred.
- Intravenous (IV) certification strongly preferred.
Responsibilities
- Provide nursing services in compliance with agency policies and procedures, state and federal regulations, and the patient’s plan of care under the direction and supervision of a Registered Nurse.
- Observe patients and report adverse reactions to medication or treatment to the Clinical Coordinator.
- Assist the Physician and/or Registered Nurse in carrying out the plan of care and performing specialized procedures.
- Instruct patients and families on diet, medication, and treatment.
- Collaborate with the interdisciplinary team to support seamless, holistic care.
