LPN Home Health (PRN Weekends)
About The Position
Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Weekend Home Health LPN to work with our Home Health team to deliver individualized care to patients in their homes in the Naples, FL area. We provide skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support as part of a seamless continuum of care. Home health visits are provided in Collier county (Naples, Marco Island, Immokalee, Ave Maria, and surrounding areas).
Requirements
- Active Florida LPN license
- Applicants must be a graduate of an approved nursing program.
- Minimum of 1-2 years of experience as a LPN.
Nice To Haves
- Home health care experience preferred.
- Strongly prefers experience using Homecare Homebase (HCHB) EMR
- Intravenous (IV) Certification strongly preferred.
Responsibilities
- Provide nursing services in compliance with agency policy and procedure, state and federal regulations, and the patient’s plan of care under the direction and supervision of a Registered Nurse.
- Observe patients and report adverse reactions to medication or treatment to the Clinical Coordinator.
- Assist the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures.
- Perform patient and family instruction on diet, medication, and treatment.
- Collaborate with the interdisciplinary team to ensure seamless, holistic care.
