About The Position
Mass General Brigham Home Care is seeking an LPN to provide compassionate, skilled nursing care to patients in their homes. This role offers more independence, stronger patient connections, and better work-life balance compared to traditional hospital settings. You will work with a diverse patient population, delivering one-on-one care where patients are most comfortable. Opportunities are available throughout Boston and surrounding communities. Requirements
- Active Massachusetts LPN license
- Minimum 1 year of acute or sub-acute nursing experience required
- Ability to travel within assigned territory
- Comfortable with electronic documentation and mobile technology
- Ability to work independently and adapt in dynamic environments
- Strong clinical, communication, and organizational skills

- Home care experience preferred
Responsibilities
- Provide nursing care and monitor patients’ conditions, reporting changes to the RN or provider as appropriate
- Assist with patient assessments, admissions, and care planning in collaboration with the RN
- Implement individualized plans of care and reinforce patient and caregiver education
- Administer medications and treatments as ordered and within the LPN scope of practice
- Coordinate care with patients, families, caregivers, and healthcare providers
- Maintain accurate clinical documentation and review patient charts to support quality care
