About The Position

This role involves providing comprehensive nursing care to children in foster care, adhering to specific guidelines and managing electronic health records. The LPN will be responsible for assessments, coordinating medical and dental care, maintaining detailed health records, and providing health education to both the children and foster parents. The position requires close collaboration with caseworkers, medical providers, and other staff to ensure the well-being and proper care of the children.

Requirements

  • Experience or interest working with children in Foster care.
  • Currently licensed as an LPN by the New York State Education Department.
  • Proof of vaccinations required.

Responsibilities

  • Administer immunizations in accordance with NYSDOH guidelines.
  • Utilize and manage an electronic health record system.
  • Adhere to ACS guidelines and 29I requirements for providing care to children in foster care.
  • Perform physical, mental health, and developmental assessments of children upon admission.
  • Arrange for initial and annual comprehensive physical examinations by a medical provider.
  • Arrange for initial and annual dental and optical examinations by agency-contracted providers.
  • Schedule appointments with primary care providers and appropriate medical specialists.
  • Maintain comprehensive electronic medical records for each assigned child.
  • Develop a written descriptive health plan in conjunction with the medical provider for each child.
  • Oversee consent issues for medical procedures and treatment.
  • Monitor compliance with appointment timeframes and follow-up services.
  • Develop health histories and abstracts for children being transferred to other agencies or schools.
  • Provide discharge summaries for youth aging out of foster care.
  • Obtain family health history from birth parents in conjunction with the caseworker.
  • Report all illnesses, accidents, and hospitalizations as required by the regulatory agency.
  • Contact medical providers, hospital clinics, and laboratories for medical reports.
  • Attend staff meetings and case conferences as indicated.
  • Review medical records, laboratory, and medical reports.
  • Provide psychiatrists with medical information, including laboratory results, for monthly medication reviews.
  • Provide health education to youth on preventive health and safety measures.
  • Train patients and foster parents on topics related to preventive health and safety measures.
  • Counsel adolescents on HIV-related information, family planning, and other sexually transmitted infections, documenting these discussions.
  • Create, implement, and maintain a care plan for each identified problem.
  • Provide daily management and coordination of appointments, supplies, and medication for foster children on the caseload.
  • Act as a resource to other staff members involved in patient care.
  • Receive and document visit summaries from all medical appointments in the record and make all necessary follow-up appointments.
  • Provide training for foster children and staff on medical conditions, health education, proper medication administration, safety precautions, universal precautions, and age-appropriate anticipatory guidance.
  • Demonstrate competency in nursing practices, including proper nursing skills, use of medical equipment and terminology, and adherence to current nursing protocols and procedures.
  • Make appropriate referrals for dental, hearing, and vision screenings and other identified problems, and follow up in a timely manner.
  • Maintain medical records in an updated (real-time), organized, and legible condition, including medication records.
  • Perform other related duties as requested by the supervisor/Director.
