LPN Community Hospice Homecare- Per Diem Days-Saratoga&Washington Counties
About The Position
The Community Hospice is seeking a Per Diem LPN to join their Hospice Home Care Team. This role involves caring for patients and families in both homecare and nursing home settings within Saratoga and Washington Counties. The position offers weekday hours from 8 am to 4 pm, with no weekends or on-call duties. The LPN will work closely with an RN Supervisor and other RNs to provide nursing and personal care services as part of a comprehensive plan of care, offering added support to patients and families. The team is multidisciplinary, including RNs, Aides, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists, and volunteers. Comprehensive orientation and training in end-of-life care are provided, along with preceptors and a supportive work environment. The organization prioritizes safety, providing training and necessary protective equipment for care delivery in clients' homes.
Requirements
- Valid NYS LPN License
- Minimum 1 year LPN experience required
- Strong computer skills
- Organization and communication skills and ability to work as part of a team
Nice To Haves
- Prior hospice/ home care experience preferred
