About The Position
The Community Hospice is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Hospice Home Care team. In this rewarding role, you will provide compassionate end-of-life care and support to patients and families in the comfort of their homes while working alongside an experienced interdisciplinary hospice team. The role offers meaningful work, comprehensive training and support, and a supportive team environment. Prioritizing safety is a key commitment, with necessary protective equipment provided. The position includes national pre-taxed mileage reimbursement and meetings/charting conducted via supplied laptop. Full benefits are offered through St Peters Health partners.
Requirements
- Current New York State Licensed Practical Nurse (LPN) license.
- 1 Year LPN experience
- Valid driver's license and reliable transportation.
- Strong communication, documentation, and organizational skills.
Nice To Haves
- 1 or 2 years of homecare and/or Hospice experience preferred
- No hospice experience? No problem. We welcome nurses who are passionate about caring for others and are committed to learning.
Responsibilities
- Provide skilled nursing care to hospice patients in the home setting.
- Monitor patient status and communicate changes to the interdisciplinary team.
- Support patients and families through education, symptom management, and compassionate care.
- Assist with implementation of the hospice plan of care.
- Collaborate with nurses, physicians, social workers, chaplains, home health aides, and other hospice team members to ensure quality care.
