About The Position
Columbia River Home Health is seeking an LPN Intake Coordinator to manage referrals, coordinate admissions, verify insurance, and serve as a key liaison between patients, families, referral partners, and our clinical team. This role is crucial for ensuring patients have access to care.
Requirements
- Current Washington State LPN license
- Healthcare experience required
- EMR and computer proficiency
Nice To Haves
- Home health, hospice, intake, referrals, or care coordination experience preferred
- Strong communication, organization, and customer service skills
- Ability to multitask and work independently in a fast-paced environment
Responsibilities
- Manage incoming referrals and coordinate admissions
- Review referrals for clinical appropriateness
- Communicate with patients, families, physicians, hospitals, and referral sources
- Verify insurance benefits and authorizations
- Coordinate seamless transitions of care
- Maintain accurate documentation and patient records
- Build and maintain strong relationships with referral partners
