The Columbus Organization is a nationally recognized provider of behavioral health and case management services for individuals with intellectual and developmental disabilities. Our team is committed to providing person-centered services that promote dignity, independence, and community integration. The Clinical Intake Specialist-LPN, under the supervision of a Registered Nurse (RN), serves as the primary point of contact for individuals with complex care needs. Key responsibilities include outreach, scheduling, and conducting initial in-person visits. The role involves performing comprehensive needs assessments and completing follow-up visits on a quarterly, annual, or as-needed basis when significant changes occur. The Clinical Intake Specialist-LPN provides options counseling, develops personalized care plans, identifies potential risks and safety concerns, and coordinates with the ongoing Case Manager to ensure continuity of care. As a vital member of the care team, they are required to report any potential abuse or neglect, collaborating with the Case Manager and leadership to address immediate concerns. Additionally, the Clinical Intake Specialist-LPN educates participants, caregivers, and family members on available care options and programs.
