LPN Clinical Coordinator – Home Health Full-Time | In Office (Newburyport) | $38–$40/hour Monday–Friday, 8 a.m.–5 p.m. | No Weekends, On-Call, or Holidays Your home health experience. In this office-based role, you’ll help connect our field clinicians, physician offices, and clinical leadership so patients receive coordinated care at home. Graduation from an accredited practical nursing program. A current, unrestricted LPN license in the state of practice. At least one year of nursing experience in a clinical setting. Strong organization, attention to detail, and follow-through. Clear, professional communication with physician offices, patients, and clinical team members. Ability to work in office and travel within the branch coverage area when needed. A dependable vehicle, valid driver’s license, and current auto insurance. Ability to frequently lift, push, pull, and support up to 50 pounds, including assisting with patient positioning or transfers and moving equipment. Spanish/English bilingual skills are a plus. What You’ll Do You’ll be a key point of contact for the documentation, communication, and follow-through that support our home health team. Support field clinicians by helping resolve coordination needs and outstanding documentation. Manage face-to-face documentation and physician orders, following up with physician offices to obtain missing information. Obtain verbal, written, and faxed orders and enter them into the appropriate system for Clinical Team Manager review and approval. Review and clear Homecare Homebase (HCHB) coordination notes and complete administrative workflows accurately and on time. Review, update, and maintain the At-Risk registry. Assist with patient follow-up appointments, preauthorizations, health summaries, and questions or concerns. Complete chart audits and verify receipt of required Medicare non-coverage notices. Protect patient and team member privacy and maintain confidentiality. Provide occasional field visits within your scope of practice, as needed. Requirements

Skilled home health nursing experience—required.
Graduation from an accredited practical nursing program.
