About The Position

Chelsey Park Health & Rehab is seeking an LPN Charge Nurse to provide high-quality clinical care and support residents’ well-being. The nurse works closely with RNs, CNAs, and the interdisciplinary team to deliver compassionate, person-centered care, and supervises and supports CNAs. The role is offered for day and night shifts, with pay of $30.40 to $33.57 and shift differentials for evenings, nights, and weekends. Requirements

• Active Licensed Practical Nurse (LPN) license in the State of Georgia or eligibility to obtain one.

• Reliability and commitment to excellence.

Nice To Haves

• Prior long-term care experience is preferred but not required.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents’ conditions and report changes to the care team.
• Provide direct nursing care in accordance with care plans.
• Supervise and support CNAs in delivering quality care.
• Maintain accurate and timely documentation.
• Ensure compliance with clinical policies and regulatory standards.
• Communicate effectively with residents, families, and team members.
