About The Position

Chelsey Park Health & Rehab is seeking a Licensed Practical Nurse to provide high-quality, compassionate, person-centered clinical care and support residents’ well-being. The LPN works closely with RNs, CNAs, and the interdisciplinary team, and supervises and supports CNAs in delivering quality care. The position pays $30.40 to $33.57, with shift differentials for evenings, nights, and weekends, management add-on pay, and weekly pay. Requirements

• Active Licensed Practical Nurse (LPN) license in the State of Georgia, or eligibility for one.

Nice To Haves

• Prior long-term care experience is preferred but not required.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents’ conditions and report changes to the care team.
• Provide direct nursing care in accordance with care plans.
• Supervise and support CNAs in delivering quality care.
• Maintain accurate and timely documentation.
• Ensure compliance with clinical policies and regulatory standards.
• Communicate effectively with residents, families, and team members.
