LPN Half Shifts (8am-12pm or 5pm-9pm Shifts)
About The Position
Apple Rehab West Haven is hiring an LPN Charge Nurse at its 90-bed, family-owned and operated facility in West Haven, Connecticut. The role plans, carries out, and oversees nursing services for assigned residents in accordance with quality-care standards, facility policies, legal requirements, medical practices, and physician instructions. The position offers half shifts from 8 a.m. to noon or 5 p.m. to 9 p.m. Starting pay is $30–$37 per hour, based on licensure and experience. The schedule includes every other weekend, and the nurse must join the LPN on-call rotation. Per diem employees are expected to work two weekend shifts per month.
Requirements
- Valid state nursing license in good standing, or be in the process of becoming a registered nurse.
- In-depth knowledge of nursing practices, medical procedures, and long-term care regulations.
- Strong attention to detail and excellent organizational skills.
- Ability to manage multiple tasks effectively.
- Collaborative team player.
- Available for the LPN on-call rotation and scheduled weekend shifts, including two weekend shifts per month for per diem staff.

Responsibilities
- Plan and provide professional nursing care that meets residents’ needs and responds to changes in their medical condition.
- Oversee nursing services for assigned residents in accordance with care standards, facility policies, legal requirements, medical practices, and physician instructions.
- Maintain professional interactions with residents, physicians, families, peers, and support staff.
- Accurately and comprehensively document information related to resident care.
- Administer prescribed medications and maintain accurate medication records in accordance with facility policies.
