About The Position

Home Health Services of Virginia is seeking a dependable, dedicated Licensed Practical Nurse (LPN) Case Manager to provide patient-centered care in patients’ homes. The position is based in Glen Allen, Virginia, and serves patients within Chesterfield and surrounding communities. The schedule is Monday through Friday, 9:00 a.m. to 5:00 p.m., with no weekend or holiday shifts. Requirements

• Valid LPN license in Virginia.
• Current CPR certification.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Reliable transportation to patient homes within Chesterfield, Virginia.

• At least one year of experience in home health or a related healthcare setting is preferred.

Responsibilities

• Provide skilled nursing care to patients in their homes according to care plans.
• Monitor patient health and communicate changes to the healthcare team.
• Administer medications and treatments as prescribed by physicians.
• Document patient information accurately and promptly in compliance with company policies and regulations.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Educate patients and caregivers on health management and preventive care.
