About The Position

Stamford Health is seeking a full-time LPN Care Coordinator to join the Medical Oncology and Hematology department in our Bennett Cancer Center. The LPN Care Coordinator has a patient management focus assisting physicians with managing care of patients with complex needs primarily with oncology/hematology diagnoses. Conduct initial and ongoing comprehensive needs assessments of patients and families. Evaluates results of care plan and readjusts priorities as patient status changes. Follows an intervention plan from the time of patient referral, coordinates care among team members, reviews patients' disposition for care, provides patient information and education, assists patients in obtaining a second opinion if indicated, transitions patients to clinical nurses and oncology nurse navigators, evaluates care and changing priorities. The role will have an impact on patient centered care through collaboration with members of the healthcare team to design and coordinate an evidenced based plan of care. This position creates a balance between individual clinical needs with the efficient and cost-effective utilization of resources while promoting quality outcome.

Requirements

  • Licensure required as a Licensed Practice Nurse (LPN) in the State of Connecticut OR Medical Assistant Certification/Licensure
  • Possesses the skills to effectively communicate, problem-solve, and maintain productive and effective interpersonal relationships while effectively prioritizing.
  • Has the flexibility to adapt to new and changing situations.
  • HS Diploma

Responsibilities

  • Provides emotional and educational support for patients and families, by matching un-met needs with referrals to a financial counselor, social worker, dietician, and a genetic counselor.
  • Identifies risk factors or barriers to care and makes appropriate referrals.
  • Serves as a resource to nursing staff and social workers on patient care issues.
  • Participates in data collection and analysis.
  • Onboarding of new referrals: Duties include facilitating the evaluation of patients by interfacing with referring physicians and specialties, including: Radiology, Gastroenterology, Interventional Radiology, Surgery, Radiation Oncology and other services as needed. Ensuring that appropriate medical records are available at scheduled appointment.
  • Coordinating appointments with providers to ensure timely delivery of diagnostic and treatment services. Monitors and evaluates the effectiveness of the case management plans and modifies as necessary.
  • Coordinating the complex management of patient at various stages of their diagnosis and treatment, collaborating with all disciplines involved in the patient's care, to assure timely progress toward the best clinical outcome in the most effective manner, providing education and resources related to, diagnostic procedures, support services, hospital processes, and other relevant information.
  • Acts as a patient advocate to assure that the patient has the knowledge to care for his/her condition and is empowered to be responsible for participating in the plan of care.
  • Maintaining communication with patients, survivors, families, and the health care providers to monitor patient satisfaction with the cancer care experience.
