About The Position

Infinite Medical P.C. is seeking a responsible, patient, and qualified Licensed Practical Nurse to review patient charts and records, ensuring that information is accurate and that documentation reflects each patient's conditions and treatment requirements. The LPN Care Coordinator provides assessment and case management services, monitors patient progress, and collaborates with interdisciplinary teams to support appropriate, continuous care and improved patient outcomes. This is a full-time, in-office position in Vinings, Georgia, with a schedule of 9:00 a.m. to 5:30 p.m. The listed hourly salary range is $30.00 to $35.00.

Requirements

An active LPN license is required.
Proficiency in conducting patient assessments and monitoring vital signs.
Excellent communication and interpersonal skills to collaborate effectively with healthcare professionals.
Ability to perform computer work, including keyboard and mouse use, for extended periods.
Ability to lift and move patients.
Ability to sit for prolonged periods.

Nice To Haves

A minimum of six months of direct patient care experience in an acute care or managed care setting is preferred, such as a hospital, nursing home, or home care.
Experience with geriatric behavioral health is preferred.

Responsibilities

Conduct comprehensive assessments of patients' medical, social, and emotional needs.
Provide case management services to ensure continuity of care and appropriate utilization of resources.
Monitor and evaluate patient progress, adjusting care plans as needed.
Collaborate with interdisciplinary teams to optimize patient outcomes.
Oversee patient electronic medical records (EMRs) to help ensure patients receive proper care.
Complete reports to reduce patient care gaps and hospitalizations.
