About The Position

Infinite Medical P.C. is seeking a responsible, patient, and qualified Licensed Practical Nurse to review patient charts and ensure records are accurate and reflect each patient’s conditions and treatment requirements. The LPN Care Coordinator conducts assessments, provides case management, monitors patient progress, and collaborates with interdisciplinary teams to support continuity of care and appropriate resource use. The role also oversees patient EMRs and completes reports aimed at reducing gaps in care and hospitalizations. This is a full-time, in-office position in Decatur, Georgia, scheduled from 9:00 a.m. to 5:30 p.m. Infinite Medical P.C. is part of a nationwide network of advanced practice providers and specialty clinicians serving residents in skilled nursing and long-term care facilities, in partnership with MedElite Healthcare Management Group.

Requirements

An LPN license is required.
Proficiency in conducting patient assessments and monitoring vital signs.
Excellent communication and interpersonal skills to collaborate effectively with healthcare professionals.

Nice To Haves

At least six months of direct patient care experience in an acute care or managed care setting, such as a hospital, nursing home, or home care; listed as preferred.
Experience with geriatric behavioral health.

Responsibilities

Conduct comprehensive assessments of patients’ medical, social, and emotional needs.
Provide case management services to ensure continuity of care and appropriate utilization of resources.
Monitor and evaluate patient progress, adjusting care plans as needed.
Collaborate with interdisciplinary teams to optimize patient outcomes.
Oversee patient EMRs to ensure patients receive proper care.
Complete reports to reduce patient care gaps and hospitalizations.
Review patient records to ensure information is accurate and records reflect each patient’s conditions and treatment requirements.
