Requirements

  • Current Minnesota LPN license
  • Strong communication skills with the ability to work professionally and respectfully with staff, residents, and families
  • Familiarity with Alzheimer's/dementia care is a plus
  • Confidence using electronic documentation systems
  • A team-oriented mindset and desire to contribute to a supportive work culture

Nice To Haves

  • Experience in Assisted Living highly preferred

Responsibilities

  • Assist each client to reach and maintain his or her highest level of performance and independence by performing assistance with activities of daily living and implementing scheduled services as needed
  • Participates with registered nurses in the development and review of care plans; participates in care conferences
  • Assist with the admission process as directed
  • Perform safe administration of medications and implementation of treatments and other therapies as directed
  • Completing documentation with accuracy and attention to detail
  • Supporting Resident Assistants through mentorship, coaching, and positive leadership
  • Managing medication orders, refills, and overall medication processes
  • Performing supervisory visits, completing chart reviews, and ensuring compliance with company policies and our Medicare Comprehensive License
  • Demonstrate effective written and verbal communication with residents, family members, staff, and other health care providers.
  • Maintain service excellence and a high level of confidentiality.
  • Actively participate in agency-wide activities and its continuous quality improvement
  • Understand the importance of seeking assistance as needed and demonstrate dependability and capability of following instructions.
  • Adhere to policies, and procedures and participate in the organization's quality improvement program.
