About The Position

This position is responsible for managing and coordinating nursing services within a long-term care facility. The role involves developing and enforcing policies, ensuring quality care, managing staff, and maintaining accurate documentation. The Resident Care Coordinator LPN plays a crucial role in the daily operations of the nursing department, ensuring compliance with regulations and providing a high standard of care to residents.

Requirements

  • Must meet all applicable state and federal requirements for this position.
  • Knowledge/proficiency of Microsoft Office Suite.
  • Be able to make independent decisions and follow instructions.

  • Capable of working with ill, disabled, elderly, and emotionally upset people within the facility.

  • Ability to lift fifty (50) pounds.
  • Moving, lifting or transferring of residents may involve lifting up to 75 pounds.

Nice To Haves

Responsibilities

  • Develop, maintain, update, communicate, and enforce facility and state-specific written policies and procedures for the nursing service department, including the safety program.
  • Maintain a current reference of written nursing material to assist the nursing service department in meeting resident needs.
  • Communicate nursing services schedules with other services to ensure continuity of resident care.
  • Develop, implement, and maintain an ongoing Continuous Quality Improvement and Quality Assurance program for the nursing services department.
  • Recommend equipment needs for the nursing department to the Director of Nursing.
  • Participate with the Director of Nursing in preparing and planning the nursing service department’s budget and staffing plan.
  • Conduct daily nursing functions in accordance with current rules, regulations, and guidelines for long-term care facilities.
  • Ensure nursing personnel comply with written policies and procedures, including the safety program.
  • Admit, transfer, and discharge residents, documenting care and services provided.
  • Participate in the orientation of new residents and their families to the facility.
  • Communicate with healthcare providers in a timely and appropriate manner.
  • Utilize nursing personnel qualifications according to resident care plans, wishes, and healthcare provider orders.
  • Conduct daily clinical and environmental rounds to evaluate resident care and report significant events.
  • Meet and resolve issues with residents and/or family members, reporting significant issues to the supervisor.
  • Participate in facility surveys as requested by management.
  • Follow clinical programs including risk management, infection prevention, quality, and staff development.
  • Complete an initial assessment and develop a service plan for assigned residents.
  • Update, review, and transcribe the service plan to identify goals, problems, approaches, and revisions based on nursing needs and new healthcare provider orders.
  • Communicate with healthcare providers to review treatment plans, complete orders, and progress notes.
  • Review resident health records for specific treatments, medication orders, diets, labs, etc., for implementation and follow-up.
  • Requisition and arrange for diagnostic and therapeutic services as ordered by the healthcare provider.
  • Communicate the department’s policies and procedures to personnel, residents, and visitors.
  • Provide written (24-hour report) and oral reports/recommendations concerning resident condition changes and other shift activities to nursing management.
  • Complete Risk Watch forms for accidents/incidents involving residents or staff and submit to the Director of Nursing.
  • Complete and file assessments and all other health record documentation as required.
  • Report discrepancies concerning physician’s orders, diet changes, charting errors, etc., to the Director of Nursing.
  • Complete clinically accurate documentation according to policy and procedures.
  • Review resident health documentation and progress notes from the interdisciplinary team to determine if the service plan is being followed.
  • Inform nursing personnel of new admissions and resident care needs.
  • Ensure rooms are ready for new admissions and greet newly admitted residents.
  • Notify the resident’s attending healthcare practitioner and family of any change in the resident’s condition.
  • Meet with assigned support personnel to plan shift services, programs, and activities, and monitor task completion.
  • Coordinate services with other members of the interdisciplinary team to ensure the resident’s total regimen of care is maintained.
  • Assist in arranging transportation for residents and other third-party services.
  • Review medication administration records for completeness and accuracy of transcription of healthcare provider orders, reporting concerns to the supervisor.
  • Dispose of drugs and narcotics as required and in accordance with established procedures.
  • Assist in the recruitment and selection of nursing service personnel.
  • Develop work assignments, schedule duty hours, and supervise nursing staff to ensure acceptable nursing standards.
  • Assist in staff development.
  • Monitor personnel performance and implement corrective actions through coaching, counseling, and discipline.
  • Collaborate with the Director of Nursing and Executive Director to resolve staff performance issues.
  • Complete timely performance evaluations as directed.
  • Review complaints and grievances, complete investigations, and work with the Director of Nursing for a plan of action and resolution.
  • Participate in planning, conducting, and scheduling in-service and orientation training classes.
