About The Position

The Clinical Care Coordinator (LPN/MA) plays a critical role in supporting providers and delivering high-quality, patient-centered care within a specialty cognitive clinic setting. This role is responsible for coordinating clinical care, performing patient assessments, managing diagnostic and treatment workflows, and ensuring accurate documentation and compliance with clinical protocols. The position works collaboratively with physicians, nurse practitioners, and interdisciplinary teams to support patients with complex, chronic conditions, including dementia and Alzheimer’s disease.

Requirements

  • Active LPN license or Certified/Registered Medical Assistant (CMA/RMA) required
  • Strong clinical skills, including venipuncture and patient assessment
  • Experience with EMR systems and clinical documentation
  • Knowledge of medication management and prior authorization processes
  • Ability to manage multiple workflows in a fast-paced clinical environment
  • Strong communication and patient education skills

Nice To Haves

  • Experience in neurology, geriatrics, memory care, or chronic disease management preferred
  • Experience with cognitive testing tools (MoCA, MMSE, SLUMS)
  • Familiarity with specialty medication workflows and infusion coordination
  • Experience working with complex patient populations and caregivers
  • Strong organizational and care coordination capabilities

Responsibilities

  • Screen and assess patients prior to provider visits
  • Obtain and document vital signs, medication histories, and cognitive assessments
  • Administer standardized cognitive testing (MoCA, MMSE, SLUMS, etc.)
  • Perform venipuncture and maintain strong phlebotomy competency
  • Assist providers during patient visits and clinical procedures
  • Provide patient and caregiver education related to diagnoses, medications, and disease progression
  • Support long-term care planning for complex patient populations
  • Order and coordinate diagnostic imaging (MRI, PET scans) and laboratory services
  • Coordinate care with radiology, infusion centers, pharmacies, laboratories, and external specialists
  • Place medication and treatment orders per provider direction
  • Manage medication schedules, refills, and prescription requests
  • Complete medication prior authorizations (e.g., CoverMyMeds)
  • Monitor laboratory and imaging results and communicate findings to providers
  • Facilitate referrals and obtain required referral authorizations
  • Review and obtain outside medical records
  • Triage patient phone calls, portal messages, and email communications
  • Answer patient and family questions regarding care plans and follow-up
  • Schedule appointments, diagnostic testing, and specialty referrals
  • Assist with incoming referrals and patient intake coordination
  • Maintain accurate, timely documentation in the electronic medical record (EMR)
  • Ensure compliance with clinic protocols, regulatory requirements, and clinical workflows
  • Track quality metrics and patient follow-up compliance
  • Coordinate closely with billing to support accurate service documentation
  • Collaborate closely with physicians, nurse practitioners, and clinical staff
  • Support clinic operations through coordination, communication, and adaptability
  • Contribute to a patient-centered, team-based care environment
