About The Position
Haven Home Health & Hospice is seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our team as a Clinical Coordinator. In this role, you will serve as a vital communication link between patients, caregivers, physicians, clinical managers, and field staff to ensure coordinated, high-quality patient care. Requirements
- Current Oklahoma LPN license
- Valid driver’s license, reliable vehicle, and auto insurance
- Excellent verbal and written communication skills
Nice To Haves
- Home health experience preferred
- Recent direct patient care experience preferred
Responsibilities
- Coordinate care across disciplines to ensure optimal patient outcomes
- Communicate changes in patient condition to physicians and case team members
- Implement and update physician orders and care plans
- Maintain accurate, timely (within 48 hours), and complete documentation
- Ensure compliance with Medicare guidelines and company policies
- Participate in on-call, holiday, and weekend rotation as assigned
- Support regulatory readiness and emergency preparedness efforts
- Assist with orientation and training of new staff as needed
