LMSW - Hospice - Full Time
About The Position
The licensed social worker administers social services to patients requiring intermittent professional services and teaches the patient, family, and other members of the health care team. Services are provided in accordance with physician orders and the established plan of care. This position requires a master’s degree from a Council on Social Work Education-accredited school of social work, applicable state certification or licensure, and at least one year of social work experience in a health care setting. State-specific education and licensure requirements may apply. Medicare home health or hospice experience is preferred. The role also requires a valid state driver license, automobile liability insurance as required by law, dependable transportation in good working condition, the ability to drive safely in all types of weather, and CPR certification for the healthcare provider.
Requirements
- Master’s degree from a school of social work accredited by the Council on Social Work Education, subject to applicable state-specific requirements.
- Certification or licensure in the state where the employee practices, subject to the listed state-specific requirements.
- At least one year of social work experience in a health care setting.
- Valid state driver license.
- Maintain automobile liability insurance as required by law.
- Maintain dependable transportation in good working condition.
- Ability to safely drive an automobile in all types of weather conditions.
- CPR certification for the healthcare provider.
