About The Position

This role is part of the Lifestyle Medicine department focused on prevention, behavior change, and whole-person health. The APP will work closely with the supervising physician, primary care providers, specialists, community health workers, administrative teams, and other virtual programs to deliver high-impact care across the health system. Lifestyle Medicine is delivered through a primarily virtual-first approach with a hybrid care model, including one-on-one virtual visits, occasional home visits when appropriate, small group discussions, and large group education sessions offered both virtually and in person. The goal of this role is to quickly understand how a patient’s habits, mindset, environment, and life circumstances are shaping their health, and to recognize the underlying patterns that may be helping or limiting their progress. In collaboration with the patient, the APP translates these insights into a clear, practical, and personalized plan that feels achievable and relevant. This action planning includes identifying key education and support needs, simplifying next steps, and helping the patient feel confident and engaged in moving forward. Care is often delivered in focused episodes, with follow-up based on patient need rather than traditional long-term continuity.

Requirements

  • Demonstrates a broad, generalist foundation in primary care, with the ability to approach patients from a comprehensive, whole-person perspective rather than a narrow or condition-specific focus
  • Licensed Nurse Practitioner or Physician Assistant (North Carolina licensure required; South Carolina licensure optional but must also maintain North Carolina licensure if applicable)
  • DEA license
  • American Heart Association BLS certification
  • Expected to obtain board certification in Lifestyle Medicine within the first year of employment

Responsibilities

  • Conduct focused virtual visits and occasional in-person or home-based visits, when appropriate, to understand how a patient’s habits, mindset, environment, and life circumstances are impacting their health
  • Work in collaboration with the patient to develop clear, practical Lifestyle Medicine Action Plans that reflect the patient’s goals, readiness, and real-life constraints
  • Use motivational interviewing and applied psychological principles to help patients build confidence, address ambivalence, and take meaningful next steps
  • Identify root causes and recurring behavioral patterns contributing to health concerns, and efficiently summarize these insights in a way that is understandable and actionable for the patient
  • Efficiently determine the most appropriate next step in care, including: Independent resources or self-directed education, Shared medical appointments, Large group participation, One-on-one visits
  • Confidently determine when clinician-level involvement is not needed and guide patients toward appropriate lower-intensity support
  • Deliver care across multiple formats, including one-on-one visits, small group discussions (generally up to 10 participants), and larger group sessions offered virtually or in person
  • Facilitate shared medical appointments, helping create a structured and supportive group environment where patients can learn from one another while still receiving individualized guidance as needed
  • Lead clear, engaging, and memorable discussions, guiding participation and helping groups function as a productive, connected experience across a range of personalities and levels of engagement
  • Support patients in building momentum, recognizing that LM care is often delivered in focused episodes rather than traditional ongoing follow-up
  • Work closely with clerical and operational staff to improve workflows, scheduling, and access, and to support the day-to-day function of a scalable care model
  • Coordinate care and communication with primary care providers, specialists, and related programs, including virtual care teams
  • Participate in student shadowing experiences, modeling effective patient communication, clinical reasoning, and efficiency
  • Use technology consistently, including prompts, artificial intelligence tools, and other emerging resources, to improve efficiency, personalize patient education, and support care delivery
  • Engage in ongoing review and analysis of patient interactions, including deeper reflection and refinement of communication, decision-making, and facilitation skills to continuously improve performance in this role
  • Complete documentation in a timely and professional manner and maintain reliability with scheduling expectations, including consistent on-time presence for virtual visits
