About The Position

Provides positive support and advocacy for individuals, assisting them in identifying choices and making informed decisions. Offers structure and support in household management, including maintaining cleanliness and establishing routines. Facilitates participation in recreation and community activities to promote socialization and relationship building. Assists individuals in improving community living and integration skills in various settings like shopping, attending events, and supporting volunteer/employment efforts. Works with individuals to learn desired recreational and social activities within their community. Provides training or assistance with mobility, adaptive aids, and public transportation. Helps develop effective communication and social skills. Operates within organizational guidelines, policies, and ethical standards, maintaining confidentiality in accordance with HIPAA and company policies. Treats clients, families, and colleagues with respect and dignity. Provides functional supports based on individualized Care Plan goals to acquire, retain, and improve independence. Helps individuals reintegrate and reconnect with their communities to encourage independence. Identifies barriers and develops proactive interventions. Educates individuals on problem-solving skills. Accompanies individuals to appointments to teach them how to utilize community resources. Serves as a role model and maintains good communication with individuals, families, and referral sources. Completes daily progress notes and other documentation, monitoring and utilizing billable hours. Conducts professional behavior at all times and participates in required training.

Requirements

  • Bachelor's Degree in related field and experience listed below OR Associates Degree with one year experience in related field and experience listed below OR High School Degree/Equivalency with three years experience in related field and experience listed below
  • Demonstrates knowledge of brain injury, accessibility to community resources and experience with appropriate training skills, assessment and evaluation skills
  • Must have a valid driver's license and the vehicle is compliant with local Department of Motor Vehicle standards such as current registration, inspection, and insurance
  • Must pass criminal background check, motor vehicle checks and a sex offender registry check

Nice To Haves

  • Certified Brain Injury Specialist (CBIS) preferred

Responsibilities

  • Provides positive support and be an advocate for the warrior
  • Provides assistance and guidance with identifying choices and making informed decisions that involve warrior's activities and life
  • Provides structure and support in household management such as: maintaining cleanliness of home environment and establishing a safe, healthy and efficient routine
  • Assists with participation in recreation and community activities in order to promote socialization and establish relationships with peers
  • Assists warrior with improving community living and integration skills in a variety of settings such as shopping, church attendance, sporting events, club participation, classroom support, volunteer/employment support
  • Works with warriors to learn how to participate in their desired recreational and social activities within their surrounding community
  • Assists with participation in recreation and community activities in order to promote socialization and establish relationships with peers
  • Provide training or assistance with mobility in home environment, using adaptive aids, and accessing public transportation
  • Provides assistance with developing effective and appropriate communication and social skills
  • Operates the program within organizational guidelines, policies, professional and ethical standards
  • Maintains organization confidentiality of information relating to individuals and their families in accordance with Health Insurance Portability and Accountability Act/company policies
  • Treats clients, families, co-workers, and outside parties with respect and dignity, honoring all values and all written and reasonably implied ethical practices of the organization
  • Based on the established goals of an individualized Care Plan provides warriors functional supports to acquire, retain and improve independence
  • Helps warriors with reintegrating and reconnecting with their communities in order to encourage a healthy level of independence
  • Identifies barriers, triggers, or changes in functional status in order to develop proactive interventions that promote continued independence
  • Educates warriors on how to develop necessary problem solving skills and strategies
  • Accompanies warriors to locations and appointments in the community, to educate them in learning how to assess and utilize their community resources and services
  • Serves as a role/mentor for warriors and maintains good communication/relationships with warrior, their families, and referral sources
  • Completes daily progress notes and other necessary documentation in compliance with organization policies and provides to own agency and to NCC Case Manager
  • Responsible for monitoring and appropriately utilizing billable hours with warriors
  • Conducts professional behavior at all times
  • Participates in training at their agency and necessary and appropriate training through National Certified Counselor
  • Communicates regularly with organization personnel regarding warrior issues/concerns
  • Participates in team meetings as requested
  • Performs other duties as assigned to meet the program goals or as requested by National Certified Counselor and own agency
  • Other duties as assigned
