About The Position

Fred Victor is seeking a Life Skills Case Manager to join their 145 Queen Housing program. This program provides safe, rent-geared-to-income housing with on-site support for single individuals experiencing poverty, chronic homelessness, trauma, social isolation, and mental health and substance use challenges. The role operates within a client-centered, harm reduction, trauma-informed, and anti-oppressive framework. Fred Victor has a long history of helping people rebuild their lives in Toronto and is committed to ending homelessness.

Requirements

  • Strong understanding of and experience in providing harm reduction and case management support.
  • Experience working with tenants facing mental and physical health issues, active substance use, history of trauma, abuse, and social isolation.
  • Experience working with tenants from racially and ethnically diverse backgrounds.
  • Extensive experience working with people who have experienced homelessness and poverty.
  • Understanding of the issues of homelessness, poverty, and discrimination facing this community and their impact on individuals.
  • Strong commitment to harm reduction, trauma-informed, anti-oppressive, and client-centered principles.

Responsibilities

  • Work with chronically homeless tenants to access and maintain alternative supported housing, focusing on challenges with hygiene, hoarding, and pest control.
  • Encourage tenant participation in developing short- and long-term goals and community development strategies using a facilitative management approach.
  • Engage with internal and external service providers to offer responsive case management support.
  • Maintain up-to-date case management files with accurate client progress notes.
  • Provide intake, assessment, and case management services using a client-centered, strength-based, harm reduction approach for clients actively using substances.
  • Coach or assist clients in developing/maintaining abilities related to daily living, self-care, crisis management, symptom management, support networks, and life skills such as budgeting and grocery shopping.
  • Assess client support needs based on indicators of stability (daily living skills, housing stabilization, finances, primary healthcare connection, social networks, etc.).
  • Draft and periodically reassess coordinated care plans (CCPs) for each client.
  • Collaborate with the housing team to provide overall tenant support and respond to crises or emergencies.
