About The Position
North Star Health is a not-for-profit, community-based health care system serving southeastern Vermont and southwestern New Hampshire. The system integrates 6 federally qualified health centers, including two dental clinics, a vision center, five school-based health centers, and a site within a local recreation center. The organization collaborates with Springfield Hospital to meet community health needs. Their health centers are Medical Home sites with the highest quality certification (Level 3) from the National Committee for Quality Assurance. Patient-centered care is evidence-based, provides high-quality outcomes, and is accessible and affordable on a sliding fee scale. North Star Health also qualifies as a National Health Service Corps (NHSC) service site, offering up to $100,000 in Loan Forgiveness. A Retention Bonus is also available. Springfield, Vermont, offers a picturesque small-town lifestyle near ski resorts and outdoor activities, with Boston two hours away. North Star Health is seeking a clinical social worker to join their expanding behavioral health program.
Requirements
- LICSW in Vermont and New Hampshire (or license eligible)
- MSW working towards licensure opportunity also eligible to apply
Responsibilities
- Provide primary care behavioral health consultations and short term interventions.
- Incorporate behavioral health interventions based on skills based treatment and facilitation of behavior change in a population working to better manage, reverse and prevent chronic conditions.
- Demonstrate professional ethics and behavior and maintain confidentiality at all times.
- Maintain professional competence through remaining abreast of current research, legal requirements, evidence-based practices, and technology.
- Consultation and contact with North Star School Based Health Centers, outside agencies and schools.
- Risk assessments (risk to harm of self and others) and collaboration with medical providers as needed.
- Execute responsibilities efficiently and in an organized manner. This would include maintaining electronic medical records.
