Licensed Vocational Nurse (LVN)

About The Position

Adaptive Home Health is hiring a field-based Licensed Vocational Nurse to deliver home health care to patients in Detroit. The role aims to provide excellent patient care while establishing clinical, documentation, and referral-relationship standards as Adaptive grows its presence in the market. The LVN will work within LVN scope under RN or clinical supervision as required and will coordinate with the office and interdisciplinary team. The full-time target is approximately 30 productivity points per week; clinicians earn more for exceeding the target, and mileage reimbursement is provided. Adaptive describes its home healthcare platform as AI-native and designed to simplify clinicians’ work through customized schedules, optimized routing, and ambient AI scribing. Clinicians may also choose after-visit voice documentation instead of recording during patient visits. These tools are intended to streamline charting and reduce after-hours documentation. The position requires a full-time Monday–Friday schedule, with some weekends as needed, and daily travel across an assigned territory. Full-time benefits include medical, dental, and vision coverage and PTO.
