Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Under the supervision of a Primary Nurse, the Licensed Vocational Nurse (LVN) will provide skilled patient care and home health services as outlined in the written plan of care and in compliance with organization policies and procedures and applicable laws and regulations.

Requirements

  • Completion of a Licensed Vocational Nursing Education Program from an accredited school.
  • Minimum of one year of LVN experience preferably in an acute care setting.
  • Current Licensed Vocational Nurse certification from the California Board of Vocational Nurse and Psychiatric Technician Examiners.
  • Valid California certification in Intravenous Therapy & Blood Withdrawal or must attend and pass Intravenous Therapy & Blood Withdrawal class within 90 days of hire.

Nice To Haves

  • Experience in a home health/ hospice setting preferred but not required.

Responsibilities

  • Under the supervision of a Primary Nurse, provide skilled patient care and home health services as outlined in the written plan of care and in compliance with organization policies and procedures and applicable laws and regulations.
  • Assist a primary nurse in providing direct care.
  • Initiate communication with physicians and managers, IPG members, HHA’s and other health care providers as needed to facilitate optimal care.
  • Instruct primary caregivers, patients, volunteers and employed caregivers in care provision based on the Plan of Care and as requested by the Primary Nurse or RN Manager.
  • Attend patient conference and participate in discussion of issues to facilitate optimal care.
