About The Position

We are currently seeking a Licensed Vocational Nurse/Licensed Practical Nurse (LVN/LPN) to join our outstanding healthcare team! As a Licensed Vocational Nurse (LVN), you will provide skilled nursing services under the direction of a RN, including medication administration, wound care, patient education, monitoring changes in condition, and documenting patient progress. You will help patients maintain independence while promoting comfort, safety, and healing. Working alongside our nurses, therapists, physicians, and caregivers, you will play an important role in achieving positive patient outcomes. In this role, you will build meaningful relationships with patients and families while promoting independence, improving outcomes, and ensuring compassionate, patient centered care.

Requirements

  • Current California Licensed Vocational Nurse (LVN) license
  • Current BLS certification
  • Valid driver’s license, reliable transportation, and auto insurance
  • Excellent assessment, communication, and critical thinking skills
  • Ability to work independently while collaborating with an interdisciplinary team

Nice To Haves

  • Previous experience working in home health, hospice, or a similar caregiving environment
  • Familiarity with KanTime Electronic Medical Record (EMR) system
  • Bilingual skills are a plus
  • Wound care/ Wound Vac experience
