About The Position

Assisted is experiencing tremendous growth and is looking for a LVN for their San Bernardino patients. They are voted #1 Home Health & Hospice Agency by Ventura County Star. This role involves working one-on-one with assigned patients, ensuring quality and safe delivery of home health nursing services. The LVN will communicate with the RN Case Manager and physician regarding patient condition and needs, and assist the RN in carrying out the plan of care. Full-time 40-hour day or night shifts are available.

Requirements

• Graduate of accredited practical (vocational) nursing program.
• Licensed as an LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners.

Responsibilities

• Work one-on-one with an assigned patients
• Ensure quality and safe delivery of home health nursing services.
• Communicate with RN Case Manager and physician, as appropriate, on a regular basis regarding changes in the patient's condition and/or needs.
• Assist the RN in carrying out the plan of care.
