About The Position

HealthFlex is a nurse-founded home health agency serving communities across the San Francisco Bay Area, North Bay, Central Valley, and Greater Sacramento Area. Licensed Vocational Nurses in this role provide care to patients in their homes, where patients and families can spend time together in a comfortable setting. LVNs work under Registered Nurse supervision and collaborate with supportive case managers. The role includes a territory radius of approximately 5–10 miles, depending on location, flexible scheduling, and autonomy in managing work hours. HealthFlex provides individualized, structured, ongoing training and documentation and technology solutions to support communication and patient care.

Requirements

• Current and unrestricted California LVN license.
• At least 1 year of LVN experience in a direct patient care setting.
• Current AHA CPR certification.
• Valid driver's license.

Responsibilities

• Conduct patient assessments.
