About The Position

This role involves providing case management services as a Licensed Social Worker. The position is full-time and focuses on clinical/technical aspects of patient care, including psychosocial assessments, care planning, discharge planning, and patient advocacy. It also includes supervisory duties, critical thinking for departmental improvement, and accurate documentation. The role requires adherence to ethical standards, financial accountability, and continuous education. The Licensed Social Worker will identify daily patient assignments, conduct age-appropriate assessments for high-risk medical patients, and identify functional, economic, cultural, and safety needs. They will assist in establishing treatment/care plans, communicate patient needs to patients and families, and provide education on diagnosis and community resources. A key responsibility is assisting patients and families in establishing safe discharge plans in compliance with regulatory requirements. The role also involves advocating for patient rights, serving on the Medical Ethics Committee, educating on self-determination, and assisting unwed mothers with paternity issues. Scheduling treatment room procedures, recording patient evaluations, and actively communicating pertinent patient information to the healthcare team are also part of the duties. The position includes performing pre-admission and concurrent reviews for Medicare and Medicaid admissions, communicating concerns to the RN case manager and physician, and working with the team to ensure safe discharges. The social worker will also assist the RN case manager with pre-admission and concurrent stay denials. Supervisory responsibilities include directing and functionally supervising the case manager assistant and mentoring/precepting new LSWs. Critical thinking involves actively seeking opportunities to improve the department, staff, and personal development, staying current with field developments, and utilizing hospital policies. Participation in Case Conferences and Plan of Care meetings is expected, along with assessing patient needs for referrals and community resources. The role requires acting as a resource for new employees and precepting as needed, maintaining professionalism, a positive attitude, excellent customer service, and fostering an atmosphere of cultural diversity. Employees are expected to communicate dissatisfaction to the Director, contribute to problem-solving, and refrain from promoting dissatisfaction among coworkers. Teamwork, cooperation, and conflict resolution are essential. The social worker will participate in departmental in-services and maintain strict confidentiality and HIPAA compliance, following the Social Worker Code of Ethics. Documentation must be accurate, complete, and free of unapproved abbreviations, completed daily, and understood for its impact on hospital payment and certification. Accurate records, including MIDAS+, time cards, and on-call logs, must be maintained. Unit financial accountability includes managing supplies, productivity, and time, with an understanding of hospital financial goals. Education requirements include attending all mandatory education and in-services, completing initial orientation and competencies, and annual/unit-specific competencies. Maintaining state-required CEUs, HIPAA regulations, CMS updates, and reviewing case management literature are also required. Mandatory BLS and health requirements (TB testing, fit testing, physicals) must be completed. Attendance is expected to be regular and predictable, including working scheduled holidays. Overtime and requested time off must be pre-approved by the Director. Other duties as assigned, adherence to dress code, assisting with special projects, and responding to emergencies are also part of the role. Requirements

  • Completion of an accredited school of social work with current active registration in the State of Ohio with good standing.
  • Masters in Social Work, Licensed as an Independent Social worker in the State of Ohio.
  • Completes 30 contact hours of approved CE during each two year renewal period consistent with requirements of the Ohio Board of Social Work.
  • Maintains continuous certification in American Heart Association’s BLS.

Responsibilities

  • Identifies daily patient assignment through hospital referral system and morning report.
  • Conducts age-appropriate assessments pertaining to the psychosocial, economic, of high-risk medical patients.
  • Provides further age-appropriate psychosocial assessment, identifying functional, economic, cultural, and safety needs.
  • Assists in establishing a treatment/care plan when requested by the patient, physician, or nursing services.
  • Communicates patient needs with patient and/or family to gain cooperation and involvement with treatment.
  • Provides education, recommendation, and/or counseling pertaining to diagnosis and community resources.
  • Assists the patient (and family) along with health care providers in establishing a safe discharge plan following patient’s rights and regulatory requirements.
  • Advocates for patient rights.
  • Serves as a member of the Medical Ethics Committee.
  • Provides education to patients, staff and public on self-determination issues.
  • Assists and educates unwed mothers on Ohio paternity issues.
  • Schedules treatment room procedures.
  • Records patient evaluation and progress information in the medical record.
  • Actively communicates pertinent patient information to other members of the health care team.

  • Performs pre admission and concurrent review for medical necessity, appropriateness of admission and level of care for Medicare and Medicaid inpatient admissions.
  • Communicates concerns to RN case manger and physician.
  • Works with the patient family and health care team to ensure safe discharge to appropriate level of care.
  • Assists the RN case manager in creating and delivering preadmission and concurrent stay denials.
  • Directs and functionally supervise the activity of case manager assistant.
  • Mentors and precepts new LSW to hospital environment and case management position.
  • Actively looks for and creates opportunities to improve department, staff and personal development.
  • Develops and demonstrates knowledge of current developments in field to maintain professional competency.
  • Utilizes hospital and departmental policies and procedures in patient care delivery.
  • Actively participates in Case Conference and Plan of Care with input from Patient, Patient’s family and other members for the Health Care Delivery Team.
  • Assesses need of patient for potential referral sources of skilled services and community resources.
  • Acts as a resource for new employees and precepts as needed.
