About The Position

Parkside Villa is seeking a compassionate and dedicated Licensed Social Worker (LSW) to join our interdisciplinary team. In this role, you will advocate for residents, support families, coordinate services, and help ensure each resident achieves the highest possible quality of life. As an LSW in our long-term care/ skilled nursing community, you will play an essential role in resident admissions, care planning, discharge planning, psychosocial support, and regulatory compliance while collaborating with nursing, therapy, dietary, and medical teams.

Requirements

  • Current Ohio Licensed Social Worker (LSW) license in good standing.
  • Bachelor's Degree in Social Work (BSW).
  • Strong communication and interpersonal skills.
  • Ability to work effectively with residents, families, physicians, and interdisciplinary teams.
  • Knowledge of resident rights, discharge planning, and care coordination.

Nice To Haves

  • Previous experience in a skilled nursing facility, rehabilitation center, hospice, assisted living, or long-term care setting.
  • Experience with Medicare, Medicaid, and post-acute care services.
  • Knowledge of state and federal nursing facility regulations.

Responsibilities

  • Complete psychosocial assessments for residents upon admission and as needed.
  • Develop and implement individualized care plans that address residents' psychosocial needs.
  • Assist residents and families with adjustment to long-term care placement.
  • Provide emotional support, counseling, and crisis intervention services.
  • Coordinate discharge planning and transitions of care.
  • Facilitate resident and family meetings as needed.
  • Assist residents in accessing community resources and benefits.
  • Advocate for resident rights and quality-of-life initiatives.
  • Participate in interdisciplinary team meetings and care conferences.
  • Maintain compliance with state, federal, and facility regulations.
  • Assist with advanced directives, guardianship concerns, and resident decision-making support.
  • Investigate and document resident concerns, grievances, and allegations as appropriate.
  • Maintain accurate and timely documentation in the medical record.
