About The Position

Southern Ohio Medical Center is seeking a part-time Licensed Social Worker to join their team. This role is vital in making communities healthier by providing exceptional care to patients. The Social Worker will be responsible for assessing patients and their families, determining and arranging for needed services, participating in case reviews, assisting with discharge planning, and providing therapy. Requirements

Bachelors required
Board Certified Social Worker, or related field (i.e. counseling) in the state of Ohio (CSW, LPCA, LCSW) within 6 months of hire
Handle With Care within 30 days of hire
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
Must be able to work in a stressful environment and take appropriate action.

Responsibilities

Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
Participates in case reviews to evaluate case management and progress.
Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
Assists with discharge planning and processes.
Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
Supervises and/or trains new staff, students, and interns.
Provides individual, family and group therapy.
