About The Position

The Clinical Social Worker provides on-demand integrated care consults to a diverse patient population in a fast-paced health center setting, in collaboration with the medical care team. The population may include pediatric, family practice, geriatric, and perinatal patients. Respond to warm handoffs from medical providers and provide on-demand consults during the patient's visit, engaging the patient the same day rather than deferring to a later referral. Conduct behavioral health screenings and brief biopsychosocial assessments, including risk assessment, and respond per clinic protocol and legal obligations. Provide brief interventions, traditional counseling, and support to patients and family members. Provide psychoeducational information as appropriate to patients, family members, and accompanying caregivers. Make referrals and connect patients to behavioral health, substance use, and community resources as appropriate. Remain current and knowledgeable about local and regional mental health and substance abuse resources, developing collaborative relationships where possible. Develop and maintain familiarity with related psychopharmacology. Collaborate alongside the behavioral health team and support the medical team to ensure holistic, coordinated care. Provide training to staff and participate in community outreach and educational activities as needed. Maintain a regular schedule at the assigned clinic in collaboration with the treatment team. Document encounters, assessments, and plans promptly and accurately in the electronic health record, supporting appropriate billing and complying. Participate in quality improvement projects, meetings, and trainings as assigned.

Requirements

  • Master's Degree in Licensed Clinical Social Work
  • Louisiana License
  • Current CPR Certification
  • HIPPA Compliance
  • Able to work as a member of a multidisciplinary care team
  • Good physical stamina and an ability to be standing most of the day.
  • Good interpersonal skills, including an ability to work well with the variety of ages, cultures, and temperaments represented among NOELA staff and patients, treating others with kindness and professionalism.
  • Commitment to demonstrating personal integrity through punctuality, honesty, an ability to follow instructions, proper attention to detail in all work matters, and a willingness to learn from others.
  • An ability to work independently, take initiative, and set priorities in accordance with the needs and mission of the clinic.
  • Flexibility to adapt to changing or stressful conditions, including unanticipated changes to working schedules or locations.
  • Adequate written and verbal communication skills for communicating coherently and professionally with patients and co-workers.
  • Conscientious of departmental and organizational policies and procedures, and able to embrace and personify the mission of NOELA.
