About The Position

Mason Creek Trans Care of Katy is seeking a Licensed Social Worker to join their team. This role involves assisting in the planning, development, organization, implementation, evaluation, and direction of the social service programs within the facility. The Social Worker will also help in developing and administering department policies and procedures, staying updated on current federal and state regulations, and making recommendations for changes. A key part of the role includes developing and implementing policies for identifying the medically related social and emotional needs of residents and interviewing residents and families to obtain social histories.

Requirements

Bachelor in Social Work (BSW)
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
Must possess leadership ability and willingness to work harmoniously with and supervise other personnel.
Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level in which they are currently functioning.

Long Term Care experience or case work a plus

Responsibilities

Assist in planning, developing, organizing, implementing, evaluating, and directing the social service programs of this facility.
Assist in the development, administering, and coordinating of department policies and procedures.
Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Director or Administrator.
Review department policies and procedures, at least annually, and participate in making recommended changes.
Assist in developing and implementing policies and procedures for the identification of medically related social and emotional needs of the resident.
Interview resident/families to obtain social history.
