The LPN provides direct primary nursing care to residents and supervises the care provided by certified nursing assistants. The role involves keeping administrative nursing staff informed on personnel development, providing feedback to Nurse Managers regarding the performance of nursing assistants, and participating in committees as assigned. The LPN demonstrates the ability to address service concerns and follow up timely for resolution, supporting the facility’s Service Excellence Initiative. This position effectively supervises and directs licensed and non-licensed nursing personnel, and is responsible for developing CNA resident care assignments and other duties as needed before each shift. The LPN promotes and maintains positive and collaborative working relationships with physicians, physician-extenders, lab, diagnostic personnel, pharmacy, nursing personnel, other departments, residents, families, and visitors. Key responsibilities include observing residents daily, reporting significant changes in resident condition to the physician, completing routine nursing documentation accurately and timely, and administering medications using the five rights of medication administration while adhering to infection prevention and control practices. The LPN also provides and receives shift reports, implements pharmacy processes, performs physical assessments, and provides wound care. They assist in the development and updating of the residents’ plan of care, participate in rounds with healthcare personnel, review orders and reports, and communicate with family members. The LPN completes accident/incident reports, notifies physicians and families of incidents, and performs various nursing skills such as phlebotomy, IV therapy, wound care, medication administration, catheter insertion, tube feeding management, oxygen administration, tracheostomy care, airway management, and physical/skin assessments. They communicate staffing problems and needs, report service and unit concerns, receive and provide end-of-shift reports, assist with resident admissions, transfers, and discharges, and report maintenance and housekeeping needs. Accurate narcotic record maintenance and validation are also required.
