About The Position

Carriage Green at Milford is hiring a part-time Licensed Practical Nurse (LPN) to work Sundays and Mondays from 2 p.m. to 8 p.m. The LPN delivers nursing care to residents in a warm, comfortable, home-like environment and works with a team of associates to support residents and build lasting relationships.

Requirements

• Current state LPN license.
• CPR certification.
• Graduate of an approved LPN program, as required by state requirements.

Nice To Haves

• One to two years of experience working as a nurse preferred.
• Previous experience working with older adults in a paid or volunteer position is highly desirable.

Responsibilities

• Accurately assess, communicate, and document residents’ status.
• Observe and report significant changes in residents’ behavior or health to the Resident Care Director, physician, family, Resident Care Assistants, and Executive Director, as required by state regulations.
• Direct and supervise the daily work assignments of Resident Care Assistants.
• Mentor, guide, and direct Lead Resident Care Assistants in all aspects of their work.
• Coordinate residents’ care needs with community providers through an effective case management process.
• Balance service demands while supporting resident independence.
• Recognize and help prevent elder abuse, neglect, and exploitation, and report concerns to appropriate sources as required by state regulations.
• Respond to emergencies and personal emergency response systems, and know when to call for backup.
