About The Position

The LPN at the Horizon Homes will manage all medical issues with direction from the RN and Physician and ensure physician’s orders are implemented, while keeping the physician and supervisor informed of pertinent information concerning the client’s response to treatment.

Requirements

  • Successful completion of a course of study that allows for licensure as a Licensed Practical Nurse in the state of VA
  • One year of experience providing nursing services as an LPN or an equivalent combination of education and experience that provides the required knowledge, skills, and abilities
  • Knowledge of the principles, procedures, techniques, trends, and literature related to physical health and intellectual disabilities
  • Knowledge of nursing best practice
  • Ability to analyze situations accurately and develop appropriate responses or techniques
  • Commitment to serving the ID/DD population
  • Ability to work in and contribute to a collaborative team environment
  • Ability to lift and transfer consumers up to an average of 50 to 80 pounds
  • Ability to demonstrate effective communication skills: verbally, person-to-person and through use of the telephone, and written
  • Willing and available to work modified schedules (weekends, holidays, etc.) as required
  • Ability to drive and must possess a valid driver’s license
  • Ability to participate in and successfully pass behavioral intervention training; includes both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed

Nice To Haves

  • Prior experience working with developmentally disabled individuals
  • Licensed Practical Nurse (preferred credential)

Responsibilities

  • Manage all medical issues with direction from the RN and Physician
  • Ensure physician’s orders are implemented
  • Keep the physician and supervisor informed of pertinent information concerning the client’s response to treatment
  • Administer medications
  • Perform medical interviews in order to obtain information and to formulate appropriate treatment plans and referrals
  • Produce, read, and relay medical lab results
  • Record data and complete documentation within established expectations and by communicated deadlines
  • Train staff to implement training programs for the individuals
  • Safely assist individuals with all aspects of physical management including equipment use, safe lifting and physical assistance
  • Respond quickly to correct situations that are deemed to be hazardous, especially to consumers
